College/University Students currently enrolled in a college/university program who require an OESC ID CARD for their placements who were born after January 1986 or already hold an OESC ID Card

B.Ed students who are graduating from their program and require a written report for the Ontario College of Teachers and for Employment who were born after January 1986 or already hold an OESC ID Card

B.Ed students who are graduating from their program or who have completed the first part of the multi session Technological Internship Program and wish to apply to the Ontario College of Teachers and for Employment who were born after January 1986 or already hold an OESC ID Card

Any individual who requires a written police report who was born after January 1986 or already holds an OESC ID Card

Consent Forms and Payment Forms are now available on-line using BackCheck’s web-based system.  An accredited and authorized  Canadian Police Service under contract with BackCheck will complete the actual record checks (name of Police Service?). 

In order to comply with recent RCMP policy, physical ID verification is required before a Consent Form can be processed.

An applicant’s Consent Form must signed and dated, at the bottom, by an authorized and responsible individual who has met the applicant and reviewed two (2) pieces of approved Identification (see below for what is acceptable), and can verify the signature and likeness of the applicant.

  1. Each applicant registers on-line and then completes an on-line Consent Form
  2. Please note: ******The Consent Form now includes a required self-declaration portion for police record offences.  This self-declaration portion is designed to speed up the Police Record Check process.  If you have any existing Offence(s) for which you have not been pardoned, you will be required to list the offence(s) on the Consent Form and provide ALL details.  If you do not complete the self-declaration portion this will indicate that you are stating that you have never been convicted of an offence for which you have not received a pardon.

    If an applicant’s Police Record Check is processed by the police and it is determined that there may be a record of conviction(s) that the applicant has not disclosed previously, the applicant will be required to submit fingerprints to the RCMP before the details of any record will be released.  If fingerprints are requested for an individual, the timelines for completion of a police record check will be much longer.

  3. Once the applicant has completed the Consent Form, he/she can then print the Consent Form (any declared convictions will not be printed out on the form), sign it and take it, in person, together with 2 valid pieces of photo-identification to an authorized and responsible agent (School Principal or designate, University or College Program Administrator, any Canada Post outlet or a Notary Public), to verify identification.   4. After written verification of ID is obtained, the Consent Form will be transmitted by email or fax to BackCheck for processing.  Applicants will have their records checked for the offences listed on CPIC databases.

  5. BackCheck will complete the police record check within one business day and transmit the results securely and confidentially to OESC.

NEW: For individuals with no record, OESC will issue an OESC ID Card valid for one year from the date of issue (changed from school year) together with a police record check report if it has been requested by the individual.  Cards and reports will be mailed to the individual within 10 business days.  OESC never reveals police record check information to anyone other than the individual applicant.

Individuals who have a record of any kind will be adjudicated according to OESC’s well-established procedures for determining if an individual is an acceptable risk to the safety of students and staff in schools.  The adjudication process may involve OESC contacting the individual for an interview and to provide information necessary for the adjudication process.  OESC ID Cards will only be issued to individuals who are determined to be an acceptable risk.
